All general admission ticket holders attending Sunday’s Premier League fixture with West Bromwich Albion have now been emailed an online health questionnaire.

The club are urging supporters to check emails and complete the questionnaire as soon as possible.

Once completed and accepted by the club, you will receive a ‘cleared to attend’ confirmation email which is needed for entry to Elland Road on Sunday.

If you have not received the health questionnaire email, please contact the Ticket Office Call Centre on 0371 334 1992 from 10am-1pm on Saturday and Sunday.
