Dennis Wise was without Jonathan Douglas and David Prutton through suspension while Radostin Kishishev and Mark De Vries were ineligible for the replay with Hereford, but the boss welcomed Ian Westlake back from injury and also recalled Paul Huntington and Ben Parker.
The two sides had drawn 0-0 at Hereford 11 days ago, but it took the League Two promotion chasers less than three minutes to rock Elland Road.
Lionel Ainsworth, who had impressed in the first meeting, worked himself an opening on the edge of the box and fired home past an unsighted Casper Ankergren to give Hereford a shock lead.
Moments later, Ainsworth mounted a raid down the right flank and his deep cross almost deceived everyone before drifting wide. And on 11 minutes, Paul Huntington was called upon to clear the danger from a Theo Robinson effort.
The visitors could be well pleased with their start, but they were fortunate to escape on the quarter-hour when a Tresor Kandol header escaped the clutches of Hereford goalkeeper Wayne Brown and was scrambled to safety.
Both Jonny Howson and Seb Carole also had shots blocked as United responded to the early goal. Carole also fired over the top after some good build-up as Hereford were pressed back.
Westlake was next to see a shot deflected to safety when he tried his luck while on 25 minutes Jermaine Beckford collected a Kandol flick-on and Brown managed to turn his shot for a corner.
Hereford's strength was their pace on the break and, after the visitors seized upon some sloppy play, Robinson wasted a great chance to make it 2-0 when his touch let him down and Ankergren got the better of a one-on-one situation.
As play continued, Ankergren remained grounded and he was replaced by substitute goalkeeper David Lucas eight minutes before the interval.
Lucas' first involvement was to save well from Ainsworth, but an offside flag was also raised to deny the Hereford winger.
United had the last word in the opening period, though, when Kandol sent a header wide following a Westlake free-kick.
Wise's men also started the second half brightly and Brown got down well to deny Beckford on 48 minutes as United looked to force a leveller. Jonny Howson also tested Brown with a low shot on 54 minutes as United pressed.
But the visitors remained a threat with their pace moving forward and Ainsworth again went close when he sent shot fizzing inches wide of the post shortly before the hour-mark. And Robinson's touch again let him down when Frazer Richardson capitalised to clear another dangerous situation.
With 24 minutes remaining, Wise added another forward to his line-up with Leon Constantine, replacing Westlake, joining Kandol and Beckford in the front-line.
And the duo combined well on 73 minutes when Brown made a good save to deny Beckford.
Yet, Hereford's reply was a quick break courtesy of Ainsworth and a low teasing ball across the goal-line which had to be scrambled to clear.
As the clock started to tick down, Hereford enjoyed more decent possession, and with seven minutes remaining Wise introduced another fresh pair of legs with Curtis Weston replacing Seb Carole.
The boss also moved Matt Heath into an advanced role as he looked to level the tie in the closing minutes.
Beckford forced Brown into another good stop and as the fourth official signalled three extra minutes there was one final roar from the Elland Road crowd.
But it wasn't enough, and United's cup frustrations continued with a second defeat on home soil in the space of week.
